One of my top clients is seeking a dedicated Home Health Registered Nurse to join our growing team.

About the Role:

As a Home Health RN, you will play a vital role in providing high-quality, compassionate care to patients in the comfort of their own homes. You will work collaboratively with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans. Your responsibilities will include:

• Conducting comprehensive patient assessments
• Administering medications and treatments
• Educating patients and families on self-care and disease management
• Monitoring vital signs and wound care (as needed)
• Documenting care meticulously
• Collaborating with the healthcare team to ensure continuity of care

Qualifications:

• Active and unrestricted Registered Nurse (RN) license in the state.
• Minimum one year of experience in acute care, long-term care, or home health setting
• Excellent assessment, communication, and interpersonal skills
• Strong organizational skills and time management abilities
• Ability to work independently and manage a caseload
• Valid driver's license and reliable transportation
• Compassionate and patient-centered approach to care
